There's nothing at first glance that sticks out. I may be reading it wrong, but it looks like you have two soundcards enabled. Unless you have a specific need for two sound cards, uninstall the drivers for one. If you decide to remove the onboard sound, don't forget to disable it in the BIOS. It may not fix anything, but it never hurts to remove one possibility. This issue might be a tricky one. It doesn't look like you've updated anything in a while, except for your video card drivers on Feb 29, which would've given you a problem much earlier. Have you installed any new hardware or software recently? If not, try some other DirectX games and see if you have any issues with them. PimpleMaze had a really good suggestion to check the event viewer. That can give some useful info, too. Here's a youtube video that explains the basics. Just copy and paste any errors/warnings that you think may be relevant. I would also suggest running a Chkdsk (link to instructions) to try and rule out the possibility of a bad hard drive sector. Hopefully that will fix things, or at least point to a possible solution. I'll take a closer look at your dxdiag and let you know if I see anything.
Did you check your temperatures while in game? There are several programs to monitor temperatures, i use gpu-z for my gpu and core temp for cpu, but there are many more. I use CCleaner (http://www.piriform.com/ccleaner/download/standard) to clean up junk files and registry. This might also help in overall system stability. Great program. (Make sure to go trough the settings what you want cleaned and what not. There's a setting activated by default to clean windows search. you have to turn this off. otherwise every time you clean the search database has to be rebuilt).
